This is an outpatient ENT role centered on evaluation and workup. The APP owns the front end of care, managing common ENT presentations, running the diagnostic process, and deciding when a patient needs the surgeon. The work is clinic based, with high autonomy and close collaboration with the ENT surgeon.
The APP manages a stepwise evaluation model. Most patients start with a general exam and a course of conservative treatment such as antibiotics and steroids, followed by a return visit. If symptoms continue, the APP moves to imaging, and based on those findings decides whether the patient needs surgical evaluation.
A core expectation of the role is clinical judgment. The APP recognizes atypical presentations that need urgent surgical referral and separates them from cases that can follow standard workup and conservative management. This front-end evaluation gives the surgeon confidence that patients reaching the operating room have been properly assessed.
This role is evaluation and workup. In-office procedures are not part of the position.
No call is currently required for this role.

The APP works with a high degree of independence while staying closely connected to the ENT surgeon. The surgeon relies on the APP to complete the workup and conservative management, then hand off cases that are ready for surgical evaluation. This division of labor is central to how Good Shepherd plans to grow ENT capacity and keep specialty care local.
Good Shepherd maintains an affiliation with Oregon Health & Science University, which opens opportunities for teaching.
The allergy service line is the clearest area for an APP to build a defined focus. Regional demand is high, and there is room to help establish allergy testing and treatment within the clinic and grow it into an ongoing service.
