Our Team

You join a tight six-provider primary care team protected by senior clinic leadership with deep operational history at the site. The Medical Director sees patients full time alongside the team, and the Clinic Director has 26 years with the organization. Culture protection is a stated priority, with every new hire personally vetted by Dr. Tabrizi and the existing team.

The APP team includes one additional advanced practice provider rounding out the four-APP staffing model, plus a recently hired physician assistant. APPs maintain their own patient panels and operate as primary care providers for patients who select them.

Dr. Tabrizi remains heavily clinical, often carrying volume comparable to the other physician despite his administrative responsibilities. He is a hands-on mentor who personally trains physicians and APPs on procedures and clinical workflow. The Clinic Director provides day-to-day operational leadership and serves as the direct point of contact for scheduling, staffing, and workflow questions.

Support Staff

The clinic is organized so each provider has consistent support:

  • Medical assistants for rooming, vitals, intake, and chart prep
  • LVN dedicated to quality measure follow-ups (currently hypertension; expanding to diabetes education)
  • In-house referrals team handling both incoming and outgoing referrals
  • Practice manager (Jessica)
  • Front-desk and central check-in staff serving all specialties on the floor

Team Culture and Collaboration

Collaboration is open-door and direct. Physicians, APPs, and support staff treat each other as equals; the Medical Director explicitly works to keep hierarchy flat between physicians and APPs. The team uses a two-step interview process for every new provider hire, with the final step a working lunch where the existing APPs and DO meet the candidate. The team has rejected candidates at the lunch step for poor team fit, and culture protection is treated as more important than filling a seat quickly.

The clinic supports Spanish-language patient care and is comfortable with multicultural patient panels. Internal and bilingual translation resources are available for patient encounters.
