Exceptional Financial Package
Compensation:
- Salary - $350,000

Productivity Bonuses:
- RVU Threshold 5833
- $60 per RVU
Sign-on bonus:
- $25,000 - 2-year forgiveness
Relocation assistance:
- Up to $20,000 – Reimbursed with receipts.
Student loan forgiveness or retention bonus (pick one):
- Student loan forgiveness - $100,000 paid out over 4 years, $25,000 per year.
- Retention Bonus - $100,000 paid out over 4 years, $25,000 per year. Paid at the end of each year.
Hospital Profit Sharing:
- This is an end of the year bonus should the hospital perform well. If so, bonuses are paid out to physicians and have averaged $13,500 over the last few years
- Paid Time Off: 36 days PTO to include: vacation, sick leave and holidays.
- CME – 1 week (5 days)
CME:
- Fully reimbursed up to $5,000
Malpractice:
- Malpractice – Per occurrence.
- Tail coverage will be paid by employer.
Retirement:
All employees are eligible to participate in the Jeff Davis Hospital Retirement Savings Plan. Through payroll deduction you may make employee pre-tax contributions up to the annual maximum the Internal Revenue Service allows. If you are under age 50 the current limit is $20,500. If you are over age 50 you may contribute an additional $6,500 as a “catch-up” contribution. The IRS may increase these annual limits each year.
- Employees who wish to enroll in the retirement savings plan should contact Human Resources, who will setup a day and time for the plan representative to meet with employees.
- The employee contribution is fully vested immediately.
- A full-time employee must be employed for five (5) years before becoming fully vested in the hospital match. Part-time or PRN employees do not receive a match.

Jeff Davis Hospital matches full-time employee contributions as follows.
- You contribute 2%, Jeff Davis Hospital will match 2%.
- You contribute 3%, Jeff Davis Hospital will match 2.5%.
- You contribute 4%, the hospital will match 3%.
Benefits:
The waiting period is 30 days of full-time active employment. Your coverage effective date is the first of the month following the waiting period.
